Thursday, June 8 from 7:30 am to 9 am Location: Jasper County Health Department. Exit off of Hwy 130 onto Clayton Avenue. Go to stop sign and turn left onto Edwards Street. Follow signs for drive through clinic. This clinic will be for 1st dose moderna or J&J single dose vaccine. Required to Receive Vaccine— • For Johnson & Johnson Single dose: 1. Read the Johnson & Johnson Fact Sheet - Click here 2. Download and complete (please use legal name) the Vaccine Consent Form 3. Download and print a copy of the ticket. 4.
