Jasper County Health Department Covid Vaccination Clinics (any individual 16 or 17 years of age)

April 27th from 2 pm to 4:45 pm Jasper County Health Department office This is a special clinic for ages 16 and 17 year olds held from 2 pm to 4:45 pm at the Jasper County Health Department office at 106 East Edwards street in Newton, IL. This will be for the first dose of the Pfizer Vaccine. The second dose will be scheduled for Tuesday April 27 while the patient is in the office. Required to Receive Vaccine— 1. Read the Pfizer Fact Sheet - Click here 2. Download and complete (please use legal name) the Vaccine Consent Form A parent or legal guardian will need to sign the Vaccine Consent Form. 3.
