April 29th from 2 pm to 5:45 pm Newton Community High School. This clinic will be for 1st shots or 2nd shots of Moderna. Required to Receive Vaccine— 1. Read the Moderna Fact Sheet - Click here 2. Download and complete (please use legal name) the Vaccine Consent Form 3. Download and print a copy of the ticket. 4. Bring the completed Vaccine Consent Form and your Ticket to your appointment. a.
