Jasper County Health Department Covid Vaccination Clinics (any individual 18 years of age or older)

Thursday, May 27 from 2 pm to 6 pm Location: Jasper County Health Department. Exit off of Hwy 130 onto Clayton Avenue. Go to stop sign and turn left onto Edwards Street. Follow signs for drive through clinic. This clinic will be for 1st shots or 2nd shots. You can register for your first shot or second shot. If you register for a first shot, you can choose either J&J single dose or Moderna first of two doses. If you register for a SECOND shot, you will receive Moderna. Download and complete (please use legal name) the Vaccine Consent Form 3. Download and print a copy of the ticket. 4.
