Jasper County Health Department Covid Vaccination Clinics (any individual 12 years of age and older)

Tuesday, June 8 from 2 pm to 4 pm Jasper County Health Department office This is a Pfizer vaccine clinic for ages 12 and older held from 2 pm to 4 pm at the Jasper County Health Department office at 106 East Edwards street in Newton, IL. This will be for the first dose of the Pfizer Vaccine. The second dose will be scheduled while the patient is in the office. Required to Receive Vaccine— 1. Read the Pfizer Fact Sheet - Click here 2. Download and complete (please use legal name) the Vaccine Consent Form A parent or legal guardian will need to sign the Vaccine Consent Form for anyone 12 years of age or older 3.
